The rhinoplasty can be performed as an open or closed technique, depending on the complexity of your case. The difference between these two approaches is small, only 5mm o skin cut in inverted V shape, which is not visible at all. The incision can be seen for almost 2-3 weeks, but the outcome is more predictable, defined. For minor tip refinement or smooth hump reduction the closed approach is advised, however for better long-term results ,the open technique is advised.
Dorsal preservation rhinoplasty is most commonly used when frontal view of the patient is normal, meaning the width of the dorsum is ideal. Slight deviation to the side also can be corrected. The problem is the dorsal hump, which can be seen in the lateral view. Here we are preserving the upper lateral cartilage and the septum connections. It can be combined with the push down method, when the bony pyramid is mobilized and the same form slides inside to eliminate the hump.
Functional or Structural Rhinoplasty is the gold standard method when after the correction of the shape deformation, like old trauma, all parts of the nose are reconstructed by using either septum or ear cartilage.
The entire nose is a network of cartilage, ligament and skin, therefore a beautiful shape can be seen. One of the main problem after rhinoplasty is that the tip elevation is going down and when you look at your nose you wish it would be just a bit higher. Teo strut provides fix long term tip elevation, with relative tip mobility, however if the septum is short, we prefer to use of septum extension graft, which provides slight more rigid tip configuration.
